Workers’ Compensation - Home Care Program
Our goal for our Workers’ Compensation Program is to enable injured workers to recover from their injuries and return to work as soon as possible. We work closely with claims professionals and physicians to ensure that we refer the most appropriate care provider to meet the needs of the injured worker. All care providers we refer are current with Workers’ Compensation medical issues, and will work to promote the best possible outcomes for patients, as well as tangible savings for employers and insurers.
We are equipped to help individuals that require care for a variety of Worker’s Compensation cases, including catastrophic losses such as traumatic brain injuries, spinal cord injuries, burns, stroke and/or other significant traumas.
In addition to ensuring the best possible fit between the injured worker and the referred care provider, we also provide regular patient progress reports as required by the stakeholders, helping all stakeholders to efficiently manage the needs of the injured worker.
We refer a diverse group of well-trained, experienced, certified, and insured care providers that can offer fluency in English, Spanish, Creole, or French.

Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) are committed to helping patients make good decisions about their care by providing them with treatment, instruction, and critical thinking about their condition. RNs and LPNs can be referred to meet your personal care or senior home health care needs, to manage medications, or to provide skilled care ordered by a doctor. The RN referred to you will conduct an initial evaluation at the start of service to ensure quality of care, and then set up a medical plan-of-treatment for the family, client, and any additional caregivers to follow.
RNs and LPNs can perform any regularly offered
service in addition to the senior home health care services listed
- Documentation of a Medical Plan of Treatment for In-Home Skilled Care (RN only)
- Assessment, Evaluation, and Documentation of an Activities of Daily Living Plan of Care (RN only)
- Regularly report to client’s Physician
- Evaluation of client's physical and emotional status - (Any licensed care provider)
- Evaluation on client's family and household to determine how they can cope and care for their loved one
- Carry out Physician's medical plan of treatment (Both RN and LPN) which may include but not be limited to:
- Monitor of complex health conditions
- Assistance with medical equipment
- Infusion Therapy (IVs)
- Peg Tube
- Colostomy, ileostomy
- Tube Feeding
- Dressing Changes
- Wound Care
- Teaching about care (injections, suctioning, new diagnosis, etc.)
- Medication Management
- Trach Care

This program is 100 percent funded by the Federal Government, meaning our services are provided at no out-of-pocket cost to the eligible veteran and their spouse. Our Welcome Aboard Package provides flexible, affordable care that helps you have a smooth, comfortable, and safe return home after discharge from a hospital, rehabilitation, or surgery center.
Upon discharge, a licensed care provider will perform the following services, as needed:
- Pick up from the facility or hospital when discharged
- Assist with discharge paperwork from the facility
- Transportation Home
- Assures safe transition from facility to the comfort
of the client's home
- Pick up essentials from store and/or pharmacy and
delivers them home
- Provides 4 hours of in-home care services

affordable cost as part of an on-going service agreement.
